YouTube Harrassment

My problem is that someone has created a YouTube account that sounds very similar to mine (Rhymemaiden1), has used a picture of me as the background for the page, has linked it to my real page, and has placed sexually suggestive material on the fake site. What can I do? I can find no way to report it to YouTube.

It should be noted that the person who set up this harassing account and cnidarious's puppetmaster are one and the same.

Go to Contact us.
Click Abuse and Policy Center.
Select 'Impersonation'. continue.
Click on 'United States, Help and Safety Tool'.
What is the issue? 'harassment'. Who is it for? 'myself'. continue.
Select 'One of the following'. continue.
Enter username to report. continue.
Select 'account is impersonating me'. continue.
Select an item from Videos & Messages. (doesn't matter what you select). continue.
Block User. continue.
Confirm. Additional notes: Write a comment stating what the harassing account is doing to you. continue.

Within 2-3 days, the account should be suspended. Youtube doesn't make it easy to report something, but in cases like this, they listen.
